REQUIRED FORMS FOR PARTICIPATION IN INTERSCHOLASTIC ATHLETICS

NO athlete can practice/tryout or compete in an interscholastic sport at JCMS unless the following forms are on file in the athletic office PRIOR to participation.

1. PHYSICAL FORM:  A current physical form completed by the student’s physician.  This form must be signed by the student, parent and physician.  KSHSAA requires that a physical form be on file at the school prior to participation.  Completed physical forms are good for the current year only and must be signed and dated after May 1^st, 2011 of the current school year that they will be participating.
2. GEARY COUNTY USD 475 – ACTIVITY/ATHLETIC PARTICIPATION FORM:  A completed and notarized Activity/Athletic Participation Form is required.  Athletes and Parents should read and indicate they understand the items addressed on this participation form.  Please initial/sign this form in the presence of a notary.  Several notaries are available at the school for school forms. 
3. JCMS CONCUSSION INFORMATION FORM:  A signed concussion form is required for participation in all interscholastic activities.  Athletes and Parents should read this form providing education on the potential for concussions during participation in interscholastic activities.  This form also addresses some possible signs and symptoms that might appear if a concussion is suspected.  Specific rules and guidelines are specified if an athlete may have suffered a possible concussion.
